I find it interesting that in topics about player ports one of the big complaints is that "in early EQ players got rich selling ports" and yet I started playing EQ in about July of 1999 (4 months after launch) and never charged for a port, I knew several other Wizards and Druids and they also never charged, and then I see threads like this that show not only weren't we alone, but it appears the majority.
Could "players got rich selling ports" be a few isolated incedents on a few isolated servers that a vocal minority used to damage a piece of the MMORPG genre?
Not wanting to derail the topic here... just stating an observation.
If I have a porter? I don't charge guildmates or people I group with often, but I advertise "Porting for donations to..." and charge the general public/strangers.
I don't port the general public for free, unless it's for a naked noob corpse run accross continents.
If I don't have a porter, I'm happy to donate the expected average amount for the level I am.
I also keep a list of non-payers, and don't offer them ports.
I typically tell people "open a trade window when you're ready to depart".
If people don't open a trade window within 30 seconds of me and they being in the same spot, and I have other customers waiting, I /tell them "I have other customers waiting" and leave if they don't respond immediately. I'll come back for them, no problem, but I don't make other people wait because they are slow.
As two boxing is approved (and by extension expected and encouraged) I expect to have a porting alt in Pantheon.
If all porting in Pantheon consumes an expensive consumable? Then I suspect everyone would charge and be expected to pay, without exception, including guildmates.
Free or price of reagents if any are required. In my experience people tend to tip even if I don't ask them to.

I believe in giving people options. There would be two choices...
1) Tell me a joke and the ride is free
or
2) Pay a fixed amount
The fixed amount would have to be on the high side to encourage people to tell me a joke. I'd rather hear the joke, but you'd be surprised how most people find it easier to pay. :(
